Student records will be issued to the student and preceptor upon receipt of documents from preceptors at the end of each trimester. They are always available from the Administration and may be requested at any time. They are kept permanently in the student file and on a disk which is updated monthly and then stored in a separate location to protect records from loss.
Student records of grades earned for academic work, on forms provided by the College, will be due at the end of each trimester and when the student submits evidence of completion of all required academic work for the degree sought. If grades are not received, transcripts cannot be issued.
Proof of clinical experience required for the Associate Degree must be submitted with the evidence of course work completed for the degree.
Student Files will include the following information, depending on degree requirements:
- All application materials.
- Evidence that the Preceptor has been paid
- Student/Preceptor Evaluation material
- Transcripts from other institutions
- National College of Midwifery Transcript
- Evidence that the NARM skill checklist was completed satisfactorily
- Evidence of licensing or certification
- National College of Midwifery Diploma
- Evidence that a final Satisfaction Questionnaire was mailed to the student
- Approved Research Proposals
- Evidence of Faculty Board acceptance of research proposals
- Library copy of the completed research paper or thesis
- Evidence of publication of research
- Evidence of presentation of research results at midwifery conference
- Records of all communication between student and College
