This information is available as part of the BSM application Packet from our downloads page

  1. The first stage of the application process for acceptance to the College is to call and speak indepth to one of the administration staff.
  2. You may then request an application packet from the College, complete it, including all materials and recommendations requested for preceptors and mail it with the $100.00 non-refundable application fee payable to the National College of Midwifery.
  3. Check that your proposed Preceptor meets College requirements. The college staff are always glad to assist you in this.

For the Bachelor of Science in Midwifery , the following will be needed:

 

From the Student

  1. High School Diploma/GED certificate
  2. Transcripts for the following courses or a statement that these will be completed concurrent with the BSM:
    1. Statistics: 3 credits
    2. Midwifery Literature and Art, 3 credits
    3. Analytical Writing, 3 credits
    4. Fundamentals of Math, 3 credits
  3. Midwifery License or Certification
  4. Student-Preceptor Contract
  5. BSM Application
  6. BSM Contract
  7. $100 non-refundable application fee

 

From the Preceptor

  1. Copy of highest Academic Degree
  2. State License or Midwifery Certification, or MD License
  3. Last 2 years of Continuing Education
  4. Curriculum Vitae
  5. Three preceptor reference forms
Once the student and preceptor files are complete, the dyad will be accepted by means of a letter.
