ASM EVALUATIONS

Student - Preceptor Evaluation Form(Word .doc) or as an Adobe Acrobat .pdf -(see the learning styles document) Send this form along with the following items for work completed this trimester to NCM within 30 days after the end of each trimester. This form is to be completed by the two of you together. Filling out this form provides you with the opportunity to 1) articulate accomplishments and problems in your teaching or learning, 2) voice satisfactions and challenges with one another, and 3) review your goals and develop a plan for the coming trimester. 4) participate in development, implementation and evaluation of the curriculum, evaluation and advancement of students, periodic evaluation of student admissions criteria, program resources, facilities, and services. 5) Participate in comprehensive curriculum review. All items must be signed and dated by the preceptor and student!

Items to send to NCM at the end of each trimester:

  • Student/Preceptor Evaluation and Plan Form
  • Student Record- updated with any new grades
  • Clinical Experience Forms-updated with new clincal experiences
  • NARM Skills List- updated with any new mastered NARM Skills

Preceptor Oversight Form - One oversight form needs to be completed by the preceptor, as part of the application, and on file at NCM for each student.

Preceptors will also be evaluated annually by NCM with a Preceptor Guidance and Direction Form. Also see Overview of Preceptor Evaluation Methods (on preceptor disk)
